Celebrating Father’s Day with Synergy Home Care

Success Story – June – Ms. Patricia Rogers
Ms. Patricia Rogers came to Sandy Lake Rehab and Care Center after a pelvic fracture. Upon admission, Patricia presented with decreased strength, endurance, pain, and was unable to complete activities of daily living without caregiver assistance.
Success Story – May – Ms. Patel
Ms. Patel came to Sandy Lake Rehab and Care Center after a hip surgery. Upon admission, she required assistance from facility staff for transfers and activities of daily living. Ms. Patel engaged in her rehabilitation journey, working closely with the facility dedicated physicians, nursing, and therapy teams on a specialized treatment plan.

Mr. Karim came to Sandy Lake Rehab and Care Center following a surgery. At admission, he required substantial assistance from caregivers for transfers, bed mobility, and to complete activities of daily living tasks.
